Purpose
To recognize members of VASHRM that have contributed to the advancement of the practices and principles of the Society through work in the profession, their community and other related organizations and have distinguished themselves through these endeavors.

Criteria
1. Member of VASHRM for 10 years or more.
2. Active member on the Board of Directors of the Society for a minimum of four years.

Selection Process
Nominations for the Lifetime Achievement Award shall be made to the VASHRM Membership Committee in triplicate no later than the 1st of December of each year. The Chairman of the Membership Committee, at least one other member of the committee and a past recipient of the Lifetime Achievement Award (when the recipient pool permits) shall review all nominations and present the top two candidates to the VASHRM board for vote at the next board meeting.

Nominations can be made by any member of VASHRM, with assistance from the nominee in completing required information, listing the contributions the nominee has made to VASHRM and to risk management/patient safety/performance improvement within their healthcare community. The nomination must present evidence of how the individual has demonstrated a lifetime, career-length performance record of multiple quality achievements as a Risk or Patient Safety Professional and describe the lasting impact of such achievements.

Nomination Portfolio
Nomination portfolios must be typed. Three copies of the nomination packet must be forwarded to the Chairman of the Membership Committee. The packet must include the following:

a. The completed Nomination Form for the VASHRM Lifetime Achievement Award.

b. Nominator's Letter of Recommendation which should concisely and persuasively indicate why the nominee should be selected, including a succinct narrative making the case as to how the nominee meets the selection criteria stated above.

c. Three Letters of Support; One from a professional, other than the person making the nomination, in the Risk/Patient Safety/PI field who is familiar with the nominee's accomplishments; Two from other healthcare or related professionals who are familiar with the nominee's accomplishments.

d. Supporting Documentation:  List of published articles, presentations, other awards, etc., not to exceed two single spaced typed pages.

e. Current resume or current CV of nominee that includes list of publications, honors, presentations and awards.

All nominations submitted, but not recognized as a recipient will be held and considered as submission for the following year. Those submitting the nominations will be contacted to obtain updated information and to verify that the nominee is being considered for resubmission. Submissions will be held and reconsidered for one year only.

Type of Recognition
Recipient shall be presented with a recognition plaque and a registration to a VASHRM Conference. Information on the Award shall be made available to the recipient's local newspaper for publication by the VASHRM secretary.

Timing of Award
The award shall be announced and presented by the President of VASHRM at the Spring meeting of the Society.

Publicizing Award
Information regarding the award will be posted on the VASHRM web site with current information for the name and address of the Membership Committee Chairman to whom nomination packets should be mailed and the dates during which packets will be accepted. Also, a list of previous winners of the Award shall be posted on the website, space permitting.

Award Name & Implementation
This recognition award is named in honor of Rose M. Tate, for her notable dedication to VASHRM and her contributions to the risk management profession. The first presentation of this award will be made posthumously to Rose Tate's family/employer at the Spring 2008 VASHRM meeting.
